I installed the module (SugarCRM Enterprise Version 7.6.2.1). Unfortunately I only see the Hierarchy lines without the Account and Contact icons. I installed the module as described and started after the installation a Quick Repair.

After the install you must do: 1) Rebuild JS Grouping Files 2) Quick Repair and Rebuild

Did you do both of them?

  There can be 2 possible reasons why the icons are not displayed properly:
  
  Old javascript cache in your browser. To regenerate the javascript cache please do a CTRL+SHIFT+R on the page.
  
  You are using a browser that is not compatible with the library that is used to display the icons of the module. Please try using a Chrome or Firefox with their latest version.
